  • National, local and state offenses (excluding Wyoming and North Dakota. Does not include Federal offenses).
  • Medicare fraud checks (Individuals excluded from participation, including claim filing, in Medicare, Medicaid and all other Federal health care programs).

Cost:  $40.00 per applicant (included applicants with multiple names/aliases).
